If you want one, draw one. As you can see by the huge amount of non-pirate submissions, most people find it's quite alright as is.

Here let me take this to a nearly insulting level of simplicity.
First you make a picture. You can either go with something as high class as Photoshop, something as cheap as GIMP, or even Fireworks, whatever tosses your salad. Hell you could even go and use something like MSPaint, as seen here.
Then you create a picture, and save as. Make note of the file name and the folder you put your picture in.
After you have completed and saved your masterpeice you then upload it to any number of free image storage sites. I myself use ImageShack, but YMMV. Once there you click on the Browse button and hunt down where you put said picture. The two pictures below will demonstrate.
After putting in the file name, you then host it. From there you are treated with a screen with all the information that you need to post said picture in a forum post. Let's take a break here and show you how the pictures in posting work.
None of the pictures here in this forum are actually uploaded onto Chefelf's server except a select few avatars and the ones that Chefelf himself uploads. They are all usually from someplace else and are just directed to your screen though linking to it. You actually get the picture from some other site, like an image dumping site or the orginal place it was posted. Be aware that hotlinking from just any site you find is frowned upon.
Ok back to the lesson. After pressing host it you are treated to a bunch of differant lines of URLs. The one we are looking for is the Direct Link, usually located near the bottom.
You then click the url so that it is highlighted, Right Click the highlighted text, then Copy it.
With the url in your clipboard you can now place the picture in any forum post you desire. Just go to the replying screen and you will see a picture of a tree.
Click it and it will bring up a prompt. Right click to paste, or just press Ctrl+V, whatever tosses your salad.
Some text will appear in your reply and with that text inside your message a picture will apear when you add your reply. Let's look at an example.
And there you go. A few points I forgot to mention. One, this topic should really be in the Suggestion box.
Second, RTFM is an accronym for Read The Fucking Manual, often a battle cry for when people on MMORPGs start spamming the general chat with such colorful phrases like "How I mine for fish?". Perfectly applicable to this situation.
And third, I really don't see the need for new smilies. We already have a multidude of smilies that we don't use that often, why add to that list. Less is more and the such.
