WTF? All the pics from ROTS, his head is way too big for his body. He looks like he could tip over at any moment....

Ah interesting. In ESB you can see what Vader looks like sans cape. The neck armor and shoulder armor are seperate pieces. Yet in Episode III it's one solid piece (and the end segments cover his shoulders are now silver instead of OT black). You could look at this as an evolution of the suit over time (being more comfortable with seperate pieces) or a continuity glitch (like the symetrical helmet suddenly turning asymetrical).

In the ROTS shot of Vader on the table, as he's facing the screen, his overhelmet looks really oversized, it dominates his whole head, whereas in the OT it seems more a part of his head, not increasing its size overall.
